Using SIPC's Online Claim Form
How to use the SIPC Claim Form Online?
All you have to do is go to SIPC Cases, scroll to and select the “Open Filing Deadline Cases
” link. Once the link is opened, select the customer or creditor claim form link, whichever is applicable, for the case that corresponds to your brokerage firm.
You may print the form and complete it manually or you may complete the form online. Once your form is complete, print it out, attach copies of relevant documents and mail in the whole package to the indicated address. If you complete the form online you may save it to your hard drive by clicking the save icon. SIPC will not retain the information or the completed form and if you close out of the SIPC website or turn off the computer without saving the information to your hard drive the information will be lost.
